What is case management?

Case Management is a person-centered approach to care that helps meet the needs of individuals and their families. Case Managers provide Therapeutic Behavioral Services-Interventions using evidence-based techniques that are goal directed and solution focused. These services are therapeutic in nature and oriented towards changing behavior.  We help you navigate health concerns, local resources, education, advocacy, insurance and funding, coordination of multiple service modalities, transportation, financial planning, and more.

Our case managers guide you or your loved one through the process of online work/credit recovery and IEP assistance.

Independent Living & Skill Development

We help develop the skills necessary to live full, independent lives. Support may include budgeting, grocery shopping, laundry, public transportation use, vocational prep, and self-advocacy. We also assist with personal goals, relationship building, ordering medication, and so much more

Creating superior health outcomes is a team effort. We monitor overall health and synchronize treatment plans with other providers and school districts.

Parental Support & Education

Case Managers work with parents to develop the skills necessary to support their loved ones at home. We also offer assistance exploring in- and out-of-home respite options, along with a variety of family-friendly community activities.
