Introducing Boundless@Home: A New Name, The Same Heart

A Program That Grows with You

We’re excited to announce that the Parent Directed Program (PDP) is becoming Boundless@Home

While our name is changing, the heart of our program remains the same—empowering individuals with intellectual and developmental disabilities (I/DD) and the caregivers who support them. The new name, Boundless@Home, better reflects the incredible diversity of families and caregivers we serve and our ongoing commitment to person-centered, community-driven care. 

Why the Change? 

Boundless@Home reflects that, along with our commitment to empowering all caregivers (parents, grandparents, siblings, friends and neighbors, too!) and creating a more community-driven, flexible support for people with I/DD needs.  This program is built to grow with the individual, whether they stay in the family home or move into their own space.  New name, same mission. 

Boundless@Home captures our core belief: that care should happen where it feels most natural, with the people who know the individual best. Whether that’s in a family home or in someone’s first apartment where they may direct their own care, this program is designed to grow and evolve alongside the person receiving support.     

What Boundless@Home Offers 

Boundless@Home continues to provide flexible, in-home support tailored by caregivers and individuals themselves, with guidance and assistance from the Boundless team. Where many staffing agencies have shortages, scheduling minimums, or high rates of turnover, Boundless@Home can alleviate those concerns.  

Families can still count on: 

  • A dedicated case manager to call for help and support
  • Paid training and resources
  • Boundless administrative support
  • A program built around your goals, values, and daily life 

We’re also looking to the future. Boundless@Home is just the beginning of an expanded vision for services that fit into real lives.  We’re building a system that adapts to your needs, not the other way around. 

A Program That Grows with You 

Whether you’re supporting a child, an adult, or someone transitioning into their own space, Boundless@Home is designed to stay with you for the long haul. And in many cases, individuals can even direct their own care. We believe in putting power in the hands of those closest to the person—and sometimes, that person is ready to take the lead.

“Our family has been part of this program for nearly 10 years, and it has truly made our lives much easier. When we were exploring provider options, we found that we didn’t qualify for enough hours to receive their services. We also considered becoming independent providers, but the responsibilities were overwhelming given our work and other commitments. This program has been a perfect solution.  It allows my son so many supportive opportunities to be out in the community and we have seen so much progress!” 

What’s Staying the Same? 

If you’re currently part of the program, you won’t notice any changes to your support—just a fresh new name and look. All the services you rely on are still here, still built around your family and your choices.
