Boundless Health Offers Free Pfizer Vaccines

Beginning June 29th free COVID Vaccines will be offered to both Boundless Health patients and the general public on certain vaccination dates from 8:30 a.m. to 4 p.m. at 445 E. Dublin-Granville Road (Building B) in Worthington. The Pfizer vaccine will be offered to all ages (including children aged 6 months to 4 years old). The vaccine for the youngest children must be administered by a nurse, not at pharmacies.
You must schedule an appointment prior to coming. No drop-ins will be accepted. Please call 614-436-7837 (EXT. 8181) to schedule an appointment. Appointments will be scheduled for 15-minute time slots, and recipients will be asked to fill out a State Mandatory form that allows Boundless Health to enter their vaccine into their state database. If you are not a Boundless health patient, you will be required to sign in with your personal/Insurance information but will not be charged.
Vaccine dates are listed below. Children 6 months to 4 years old will receive three separate appointments. The second dose will be at least twenty-one days after the first dose and the third dose is at least eight weeks after the second dose.
Children ages 5-11 and adults ages 12 and up will receive two separate appointments at least twenty-one days apart.
