Boundless Health was created to meet the primary and preventative healthcare needs of individuals and families—especially those who often face barriers in traditional healthcare settings. Our Services 

Boundless Health offers an expanding range of primary and preventative care services for patients of all ages. Whether you’re coming in for a routine visit or managing an ongoing health concern, our team is here to support you every step of the way. 

Our services include: 

• General visits, such as annual physicals, well- and sick-child visits, and family planning 
• Medications and vaccinations, including prescription refills and routine immunizations 
• Screenings and testing for chronic conditions like diabetes and high blood pressure, along with rapid tests for flu, strep, pregnancy, A1c, and more 
• Onsite lab services, including blood draws and urine collection 

Preventative care is a cornerstone of our approach. Through health promotion and patient education, we help patients take an active role in maintaining their health and preventing illness. 

When specialty care is needed, our providers will work with you to coordinate referrals to trusted specialists supported by your insurance plan, helping make the healthcare system easier to navigate.
